Healthmark is pleased to provide information regarding the prices of our health care services and hospital policies, as well as links to insurance providers and other important information.Your hospital bill is determined by the length of your stay, the type of room, the medications and hospital services ordered by your physician. The charges for the services of your physician, emergency room physician, consulting physician and other health care professionals are not included in your hospital bill. Those professionals will bill you separately.

Healthmark Regional Medical Center (HRMC) charges are set according to the service(s) rendered regardless of the patient-related factors such as insurance, medical status, etc. “Charges” mean the regular rate for a service which pertain to both insurance beneficiaries and uninsured patients who receive the service.

Patients can request a Good-Faith Estimate* of charges for non-emergency care. HRMC will provide the Good-Faith Estimate within 7 days of the request as per Florida Statute 395.301 (1) (b)1. Requests for Good-Faith Estimates must include the service and/or procedure that the estimate is related to and your contact information.  In order to obtain a Good-Faith Estimate:

  • Contact the HRMC Business Office Manager at (850)951-4599. Make a personal request at the HRMC Business Office, Healthmark Regional Medical Center, 4413 US Hwy 331S, DeFuniak Springs, FL 32435, or
  • Mail requests to: HRMC Patient Inquiry, 4413 US Hwy 331S, DeFuniak Springs, FL 32435. Note: If you mail your request, the Good-Faith Estimate will be provided to you within 7 days of the date the request was received at HRMC.

 

A Good-Faith Estimate* is an estimate of charges only.

  • The Good-Faith Estimate is based on reasonably anticipated charges by the facility for the treatment of the patient’s or prospective patient’s specific condition, and is not adjusted for potential insurance coverage as per Florida Statute 395.301.
  • HRMC makes no guarantees regarding the accuracy of the pricing information provided in the estimate.
  • The Good-Faith Estimate only includes Healthmark Regional Medical Center’s Hospital or Rural Health Clinic charges.
  • All estimates are based on information provided by a prospective patient and do not include, among other things, any unforeseen complications, additional tests or procedures, and non-hospital related charges, all of which may increase the ultimate cost of the services provided.
  • A final bill for services rendered at HRMC may differ substantially from the information provided in the estimate, and HRMC shall not be liable for any differences. The actual bill for your health care services is determined by the service(s) provided, length of your stay, the type of room, and medications ordered by your physician and received during your treatment.
  • The charges for the services of your physician, consulting physician, radiologist, pathologist, and other professionals are not included in your hospital bill. These professionals will bill you separately.
  • Physicians that provide services at the Healthmark of Walton, Inc. may or may not be in-network with your insurance company. Please contact your health insurer or health maintenance organization for specific physician and provider coverage.      

 

Please contact your health insurer or health maintenance organization for additional information concerning cost-sharing responsibilities under your particular insurance plan such as coverage-exclusions, copay, coinsurance, deductible amounts, and in- and out-of-network coverage and cost-sharing responsibilities. Healthmark Billing and Collection Policy

As a courtesy, Healthmark Regional Medical Center (HRMC) will bill a patient’s insurance if the insurance information is provided in a timely manner. If the patient or guarantor fails to notify HRMC of their insurance in a timely manner, the guarantor may be held liable for the full amount of the charges.

Once insurance has adjudicated the claim, the outstanding balance will be billed to the guarantor.   If the patient does not have insurance, the charges will be billed to the guarantor on the account.  The guarantor is the person that is financially responsible for payment of the services provided to the patient.  Generally, this person is the patient, but it could also be the patient’s parent, guardian, etc.

Healthmark of Walton, Inc. will utilize an indiscriminate and consistent methodology to bill patients. Patient billing will consist of:

  • an itemized statement will be mailed to the guarantor within seven (7) business days of request and after discharge and medical coding,
  • a guarantor statement will be mailed to the guarantor within a month of the account being put into self pay status (self pay status means that the balance remaining on the claim is the responsibility of the guarantor),
  • after ten (10) business days, if the account has not been paid in full or put on a payment plan contract, a second invoice will be mailed to the patient,
  • after ten (10) business days, if the account has not been paid in full or put on a payment plan contract, the account is placed into a bad debt status and is reviewed for further collection action, including:
  • Attorney notification letter of possible further collection and litigation action,
  • Turn over the account to a collection agency, Pursuit of judgment, and/or Pursuit of garnishment.            

As a courtesy, HRMC will provide patient’s with the opportunity to set up a payment plan contract. The amount of the monthly payment is established using the following guidelines:

Account Balance Minimum Payment (per account)
$25.00 to $300.00 $25.00 per month
$301.00 to $750.00 $65.00 per month
$751.00 to $3,000.00 $125.00 per month
$3,001.00 and up Contracted amount divided by 24 (2-year payment plan)

 

Patients and/or Guarantors can request a review of billed charges. HRMC will provide a response to the inquiry within seven (7) business days of the request as per Florida Statute 395.301 (5).  Requests for Bill Review must include the item that the question is related to and your contact information.  In order to obtain a Bill Review:

  • Contact the HRMC Business Office Manager at (850)951-4599. Make a personal request at the HRMC Business Office, Healthmark Regional Medical Center, 4413 US Hwy 331S, DeFuniak Springs, FL 32435.
  • Mail requests to: HRMC Patient Inquiry, 4413 US Hwy 331S, DeFuniak Springs, FL 32435. Note: If you mail your request, the Bill Review response will be provided to you within seven (7) business days of the date the request was received at HRMC.          

 

Healthmark Provision of Patient Charity Care, Discounts, and Financial Assistance Policy (BO-05)

Healthmark Regional Medical Center (HRMC) is a for-profit hospital which pays taxes and therefore does not routinely provide charity care except under the specific recommendations/guidelines of Medicare, Medicaid, or any other jurisdictional requirements. However, any patients requiring care who are subsequently determined to fall within the Federal Poverty Guidelines will be given consideration by Administration to be classified accordingly. HRMC Registration Clerks and Patient Account Representatives (Collection Clerks) will provide Applications for Financial Assistance to patients upon request. Completion of the application does not guarantee approval of financial assistance. Patients must continue to make payments against their outstanding accounts while the accounts are under review. These payments toward outstanding balances are not refundable.

HRMC will not “use discounts to induce referrals for items or services reimbursable by any federal health care programs, will not offer discounts to influence a federal health care program beneficiary’s choice of provider, and will not provide routine or automatic discounts on co-payments, deductibles, cost-share, or charges to the uninsured.” Registration Clerks and Patient Account Representatives shall provide Applications for Florida Medicaid. HRMC Patient Account Representatives (Collection Clerks) shall submit Medicaid Applications through the internet to expedite the application process.  Submission of the application does not guarantee eligibility for Florida Medicaid benefits.
